Beware of neck pain after Covid-19!

In the ultrasonography performed to investigate this pain, we detect that the pain is concentrated on the thyroid gland in most people. On ultrasonography, we observe that the thyroid gland is darker in color and its structure is impaired. This condition is called viral thyroiditis. We can say that it is a thyroid inflammation, that is, inflammation caused by the coronavirus.”he said.

“DAMAGES THE THYROID Gland”

Mentioning that viral thyroiditis has increased significantly during the pandemic period, Prof. Dr. Onur Sıldıroğlu said, “Also, this situation continues for a long time. Even if the pain subsides in the control ultrasonography, we observe that it damages the thyroid gland, changes in thyroid hormones depending on the degree of damage, and causes hypothyroidism in some patients. Accordingly, long-term, sometimes even life-long thyroid hormone replacement treatments may be needed.

“This situation, which we call viral thyroiditis or subacute thyroiditis, which we rarely see before the pandemic, is a situation that is seen especially in those who have viral infections of the adenovirus or enterovirus type in the winter and spring months. During the pandemic period, it started to be experienced due to the coronavirus. It is thought to be caused by the overstimulated immune system attacking the thyroid gland, that is, its own tissue, rather than the virus spreading directly to the thyroid gland and causing infection.”

“THYROID INFLAMMATION CAN BE MIXED WITH CAUS VESSEL INFLAMMATION”

Emphasizing that it is necessary to pay attention to the carotid vein inflammation that is confused with thyroid inflammation after Kovid-19, Sıldıroğlu said, “In addition to viral thyroiditis that increased after the pandemic, pain due to inflammation began to be seen frequently around the carotid artery, which passes by the thyroid glands, which is popularly called the carotid vein. In medical language, we call the inflammation of the carotid artery carotidinia.

This situation, which we observed once a month or once a year in the ultrasonography we did before the pandemic, peaked with the pandemic. This overlooked disease can be detected more clearly with new technology ultrasonography devices. After evaluating with ultrasonography in terms of subacute thyroiditis in the patient who presented with neck pain after Covid-19, if the thyroid gland is normal, we examine the carotid artery with Doppler ultrasonography if the patient’s pain is adjacent to the thyroid gland and spreads to the sides. Since the jugular vein is adjacent to the thyroid gland, the pain in this area can be confused with subacute thyroiditis during examination.

Carotidine can be found alone or it can be a stimulator of large vessel vasculitis. We know that Covid-19 infection causes vasculitis, known as vascular inflammation. In some patients, contrast-enhanced MR angiography can be performed on neck veins to investigate this situation and, if necessary, to determine the condition and length of the involved vessel.

prof. Dr. Sıldıroğlu emphasized that it is extremely important to do neck ultrasound in terms of not neglecting neck pain after Covid-19, investigating the underlying causes and planning treatment.
